PHMC's Forensic Services coordinates criminal justice substance abuse treatment for nearly 10,000 individuals each year through nine assessment and case management services in critical programs. We serve youth, adults and families involved in a wide range of criminal justice initiatives.
Potential data breach involving personal information of 23 clients served by Public Health Management Corporation’s (PHMC) Forensic Services.
On January 9, 2017, Public Health Management Corporation (PHMC) was notified that a security breach may have occurred involving information used by PHMC’s Forensic Services team. A breach may have resulted from the theft of a workplace laptop, which was stolen from an employee’s locked car. Information from various case notes included dates of birth, social security numbers and home addresses. Most of the individuals who may have had their personal identity breached have been contacted. Numerous and concerted attempts to contact the remaining 11 individuals have been made, including site visits to last known addresses and the delivery of registered letters. Unfortunately, those attempts were not successful. PHMC has enforced policies to prevent unauthorized access to member information.
